Widget Not Showing Input Box

    Hi,

    I am a web designer. We are using a regular WordPress site version 4.1.1 and wanted to try this plugin. We are not going to use the ReadyGraph feature. I want to display the email input form in a widget, but even though the widget settings are set to “Show Complete Form”, the form still isn’t showing up. Any suggestions?
