WP 4.1, and a custom theme which basically is… a stock theme heavily customized in cosmetical (CSS) terms, but not much more (please see the blog link in my profile).

sorry for the long delay. I have tried everything: installed the plugins that usually create this issues (All in one SEO, WPTouch, Broken Link Checker,…), used featured images, custom fields, custom size thumbnails, uploading the file, inserting it via link,… The problem is that you get really weird sizes that our plugin would never create on its own: 40×39, 12×40…this one felt like something needed for mobile but unfortunately I could only do tests with the free version of WPTouch and couldn’t create any duplicates with it. Not saying it is WPTouch fault, just the first thing that pops to mind with such small sizes. Then I tried changing the thumbnail sizes that WP creates by default (via Settings->Media), which we then use for posts, still nothing. I even tried using different but still standard WP themes. Nothing.
I am running out of ideas to be honest. BTW – what kind of names do these duplicate/new images/thumbnails get?
