We need help

We are struggling to implement the following:

When users sign up with us, they can choose between one of the two roles: Customer and Agent.

We have set up user roles in Ultimate Member Pro:

And set up Radio user role form to display these 2 choices:

And we have also set up the conditional logic:

However, it does not show the user role choices. And conditional logic does not seem to work either:

We need to resolve this issue ASAP. Earlier, when we finally get someone to answer our questions after waiting for 2 weeks, we provided a lot of information.

The moderator of this forum promptly deleted our posts as he was concerned we went too far. (I am eternally grateful that he was so quick to act this time, although he was not responsive in the past 2 weeks, and it does not seem to matter the solution does not work for us.)
