The sidebar widget works fine; it’s just the page code that is not rendering.
Can you retry with the latest version? I’ve updated the plugin with several bugfixes.

The problem is that since WordPress 4.4 you cannot use spaces in shortcodes, so you have to update the shortcode to:
[jQueryArchiveList month_format=number showpost=1 showcount=1 ex_sym=+ con_sym=- fx_in=fadeIn type=page]
