I have the same problem. I think it is a bug when you are using gravatars with a fallback image for when no gravatar is found. Try setting the fallback image to a different option, e.g. Mystery Person.
This ought to be very easy to fix properly though, so hopefully they will update the plugin soon.

I have this problem on one of my client’s website too. When I added two new testimonials, only one would save. The moment I added a second one, the first one would disappear from the list (I tried several times, but only one could be added…). In my testimonial list I see the first testimoinal, but also this error message further down:
Warning: DOMDocument::loadHTML(): Empty string supplied as input in /home/coachi28/public_html/wp-content/plugins/easy-testimonials/include/lib/gp-testimonial.class.php on line 950
The gravatar was already set to “mystery person” on this site. I changed it to “Blank”, but didn’t solve the problem.
Hopefully the plugin authors can solved this somehow, as I really love to keep using this great plugin.
