Video Format Post – No box to paste video link or embed code

Instructions for posting video format as are follows:

“This is the video post format. All you need to do is paste the video link (or embed code) in one of the two fields below the large text box.”

However when I choose video as a format, no boxes appear below the large text box. I tried to edit a previous post, also tried a new post. Everytime I choose video format, I don’t see a box to paste the video link or embed code.

What I see below the large textbox are:
– Post options for primary sidebar and secondar sidebar, and layout options
– Revisions
– Excerpt
– Send Trackbacks
– Custom Fields
– Discussion
– Slug
– Author
– Comments

I disabled all plug ins.

This is a fresh install of the Hueman theme.
