VERY simple order form/shop

Hello peeps 🙂

I was wondering if someone could direct me to a very simple shop which just lets me add a table with like tre columns (one with image, one with a short description and one with a short form where the user just fills in the quantity they want for the desired product in the image).

I know this might be solved with a contact form plugin or such, but none of them lets me design the page as I want and none of them supports inserting a picture like this inside the form.

Basically I would like kind of a traditional list view from a webshop, just simpler, without links and just quantity support.

Any ideas? Thanks 🙂
