Using your form for Conntact Form 7

Harish, you helped set up my Mailchimp sign up form yesterday. I need help to get my Contact Form 7 working in the same way ie. I must receive the email and when someone ticks the Subscribe to newsletter checkbox the contents of Contact Form 7 should go to MailChimp also. As you did yesterday, the radio buttons of the MaipChimp sign up form must also be in the contact form 7 so that subscribers are sent to correct mailchimp lists.
Currently I tried using some plugin called Mailchimp Extension for Contact Form 7 but as you can see its not working on my contact form at http://shobhaponnappa.com/contact-me-drop-me-a-line-with-your-query-or-feedback/. The drop down list is working with Mailchimp but not posting and email to me. The radio buttons send email but do not send subscribers to mailChimp.
What is the way to integrate your plugin with Contact Form 7?
