   with your plugin activated users must enter their username and password
   twice to log in.

 * We cannot help to troubleshoot the issue if the plugin is not active.
 * If you re-enable W3 Total Cache, please add /login/ to Performance>Page Cache
   >Advanced>”Never Cache following pages” field, save all settings and purge the
   cache and see if the problem persists.
