User feedback form

I am looking for a plugin that will allow a user to input their feedback into a field like when posting this to a forum.

Ultimately what I am trying to do is use the survey plugin I have and post a few questions and at the very end put in a field for text input. Once the user pushes submit it would either be emailed or kept in the comments part of the page.

Does anyone know of a free plugin that would do this or would you suggest that the comments section just be used here?
