    Firstly, I want to mention, that you can easily achieve your desired result with the Checkbox (Multi) question type of our Survey Maker plugin.

    In order to achieve your desired scenario, you can go to the Surveys > particular survey > General Tab and choose the Checkbox (Multi) question type from the list.
