The second issue sounds like your site is using a “reverse proxy”, so Wordfence cannot see the visitors’ actual IP addresses. You will need to set the option “How does Wordfence get IPs” on the Wordfence Options page. Depending on what software your server is using, you probably need to choose the X-Real-IP or X-Forwarded-For option, or if you use CloudFlare, then choose the CF-Connecting-IP option. After setting the option, you can verify it is working by looking at the Live Traffic page, and visiting the site in a separate browser where you are not logged in, and verify that your own IP appears in your own visits. (If Live Traffic is disabled, try logging in using the second browser, since logins and logouts are still recorded.)
I couldn’t see the screenshot from the first issue, so I’m not sure if that was related or if there was a separate cause. Can you try the steps above, and let us know if you’re still having the first issue?

Since you block access to wp-admin you are actually blocking yourself from too.
I suggest to whitelist your IP(under other options), save and try again. That will work if you have a static IP if not once your IP changes you will get blocked again.

@k2300: Thanks for the additional details. I don’t mean that you should add any type of proxy, but rather that your host is already running a proxy. If you try changing the “How does Wordfence get IPs” option to the choice that says “X-Forwarded-For”, then check your Live Traffic after some new visits come in again, it will most likely fix the issue. If not, you can try the “X-Real-IP” option in the same field.
The blocked URL option is intended for pages that don’t exist (to block bad bots that test a bunch of known bad URLs), so in this case, it should not have an effect with the way it is currently set up. Blocking wp-admin is also bad for a number of plugins that use wp-admin/admin-ajax.php, including Wordfence, so we generally don’t recommend doing that.
