Urgent: Issues with Form Submission

I created a registration form with a field for file upload, but each time someone fills the form, it loads and stays on the same page. So I created another form without a file upload field and it submitted successfully. The form I need has to have a file upload field because I need users to be able to upload some documents.
How can I fix this?
