Uploads not appearing in Media Library or UGC

Hi, I’ve been using this Plugin without any problems since June of last year on a private development site.

I’m not sure why but suddenly the uploader isn’t showing content in the UGC or the Media library. I do get a message via email that the song has successfully uploaded, but it’s not anywhere I can find. I have 3 pages each with basically the following code to submit one song each (domain name changed for privacy):

In January, we moved the site to a new domain. I used Velvet URLS to get the updated URL in place throughout the site. I reset the permalinks. All other aspects of the site are working fine. I can upload things using Gravity Forms in another part of my workflow, but the Frontend Uploader isn’t uploading properly.

This is a critical item for my client project, so any help would be greatly appreciated. :~)
