Uploading and Sending of files not working

I’m desperately trying to get N-Media’s form to work on this site and the files are just not taking. I read the other comments here in the forum and I already have a data name assigned as well as jpg,jpeg,pdf,tiff,tif,pict,pct,png as my file types. So why can’t I get this to work?!
