Upgraded plugin, was forced DB update and now forms disappear

    I was able to use the form shortcode in a text widget. Then add the text widget to the sidebar.

    Shortcode looks like this: [ccf_form id=”YOURFORMIDNUMBER”]

    This worked but the form had to be restyled as the CSS scope was pointing to a different widget.
