Update to 4.0.41 – disappearance of existing forms

    jumpjam.co.nz uses the Fast Secure Contact Form to publish 23 forms on its website.

    Last week the plugin was updated to 4.0.41 resulting in all 23 forms disappearing.

    The website uses the Customizr theme version 3.4.21 and WordPress version 4.5.2

    As a temporary fix we have reinstalled version 3.1.9 from a backup and all the forms have reappeared.

    Any advice about how to resolve the problem so that we can keep everything up-to-date, would be welcome.
