 * I have a number of entries in my log like the above, from two different IP addresses.
   What causes the username to be “Unknown System” when a login in occurring?

 * Hello Greg,
 * Thank you for showing interest in our plugin. As such the plugin should be able
   to report the username used for the login in most, if not all of the cases. If
   it is failing to report it, then something is “wrong”.
 * Do you have any plugin or use any custom code for users to login?
 * Also is it happening for all the users or just for some of the users?

 * We’re using the Theme my Login plugin.
 * It is a small number of logins, most do show the correct user.

 * Hello Greg,
 * Thanks for the update. Considering the issue is only happening with some of the
   users, then can you point out some differences between the users for whom the
   plugin reports the correct username and for those it doesn’t?
 * For example, maybe they use a different login page, or have a specific role or
   something similar.
