I don’t know exactly what’s causing it at the moment, but I’ve informed the right people that it’s happening.
I can say that the file mentioned above is part of a metabox library that BadgeOS utilizes. Due to this component being 3rd party that we’ve integrated, it is possible that other plugins have too and that could cause potential conflicts. Especially if the same javascript files are being added multiple times. This link that I got from googling the error is where this thought comes from http://support.dev7studios.com/discussions/caroufredsel/ZN-026319
So one thing to check for is if you have a theme or plugin that is using https://github.com/jaredatch/Custom-Metaboxes-and-Fields-for-WordPress as well.

this is happening only after i install badgesOS and when i try to insert image in any post,
I have other plugins installed but none of them are clashing with this plugin,
Only that basgesOS is clashing with core wordpress image insert feature it seems, I may be wrong,
Should I give u access to admin area?
I’ve heard that there are a couple issues with the Canvas theme from Woothemes, so I wanted to make sure we weren’t working with that one.
