Uji popup is not working on Envision theme

Hello, I’m working on this site http://www.policyforlife.com/policy/amulya-jeevan-2/# when you click on “calculate premium” which is responding properly and not showing results. I’m using envision theme which is not supporting external javascript/jquery scripts. I’m newbie to this area. could you please help me out. Thanks.
