If you deactivate the plugin then the shortcodes you added to your form will no longer execute, and will instead show up as text. This is the case with any plugin that adds shortcodes.
Rather than post a 1-star review with no detail, why not post a support request including your error messages? I will be more than happy to help.

Hi Angus,
I deleted all of the shorcodes before I deactivated the plugin. Still many many errors on deactiation. Eventually I had to (1) Install and activate your plugn. (killed the errors) (2) go into the backend via FTP and delete your plugin (3) on restarting word press your plugin was finally GONE and no errors.
ie – deactivation of your plugin caused the errors
