If you edit the download item, do you see the thumbnail image URL there?
When I would like to add the download, I cannot just say select file. I have to insert the URL.
After I have selected an image, there is shown a little square next to the two bars (select/remove image). When I publish the square dissapear – and it is like no image is added.
I’m having the same experience with a new install of Simple Download Monitor. I tried uploading a new image and using one from the Media Gallery with no success.
The image that is displayed is the “undefined” placeholder image.

Do you see a value in the “thumbnail image URL” field after you upload the image?
After the page is save the placeholder image is gone and the selected image is not displayed.
That is how it looks in my place, too

Looks like the image you selected is giving a 404 error. Is that image accessible from the browser’s address bar?
Thanks for the reply. I tried testing with an image available via the web and by uploading multiple images. Unfortunately they all resulted with the same ‘unlisted’ image.

Please upgrade to the new version 3.2.0 and you shouldn’t see this issue anymore.

My test results with the 3.2.0 update:
Pasting a link to an image works and the thumbnail loads as a 75×75 image on the corresponding page for the file.
Clicking “Select Image” and then choosing an image still displays the undefined image placeholder — https://dl.dropboxusercontent.com/u/518644/Screen%20Shot%202015-02-27%20at%207.22.36%20PM.png
I also attempted to upload an image and it resulted in an undefined image as well.
OH! I just found a way to add the image as a thumbnail by accident.
Instead of clicking “Select Image” I clicked “Add Media” after I clicked “Insert into post” I saw the image appear in the “File Thumbnail” area. The only quirk I noticed with this workflow is that the URL to the full size of the image was inserted and not a URL to the Medium or Thumbnail.
I should note that the cursor must be in the dialog box for “File Thumbnail (Optional)” before clicking “Add Media”.
