Thank you for using Paid Memberships Pro, this sounds like a site specific issue.
Could you please share a list of your active plugins, often there are plugins that ‘fight’ for priority to the login page and things can get confused with this process.
Do you have any other plugins that may offer password reset functionality or frontend login forms?
Does this issue persist if you set the Paid Memberships Pro log in page to “Use WordPress Default”?

Please can you retest this with only Paid Memberships Pro active, and set your theme to TwentyTwentyOne?
You may reactivate your plugins (one at a time) and retest the password reset functionality. The last plugin or theme active before this issue re-appears will be the conflicting plugin.
I’d recommend in starting with JetPack as they may do some things to the login process.
