Them-my-login-Custom not working

I am disabling temporarly TML due to de fact that I need to use custom filed in the registration form.
I was not able to make the theme-my-login-custom.php to work.
No matter what coode I insert there it does not show.

I will re-enable TML once this is solved because I am loosing visitors due to this limitation.
Plese help when you can
