Typical form use would be to place the shortcode on a custom confirmation page you create for account deletion, then link to the confirmation page from somewhere appropriate on your site (e.g. a profile page).

I would not suggest putting the password confirmation form on a profile page directly.
Now, everything seems to get in place. I understand the concept.
So, the confirmation page must be created by me. Then, place the code there. When user clicks delete, he is redirected to the confirmation page and enter password.
If I don’t choose use form instead of link, then when the user clicks delete, the pop-up appears and tell the user confirm deletions. Here there is no need to enter password? That’s where I’m getting confused a little because in the settings I checked confirm password but if I setup like this, the account is deleted right away after clicking ok in the pop-up window.

The confirm password checkbox is not part of the Shortcode usage of Delete Me. There are two implementations of Delete Me. Automatic placement on the WordPress “Your Profile” page and a custom placement using a shortcode.
