Tag appearing twice

    On my site I have several tags, one of which is holidays – customs & traditions. I have 16 posts tagged with this tag.
    Then I tagged two other posts with it, but this strange thing happened: I nog have two (!) tags called holidays – customs & traditions, but both with another slug.
    I’ve deleted the newest tag from the two posts, then deleted the tag and than added the only holidays – customs & traditions post there was left to the two posts and then suddenly the other tag, with the same name but a different slug, was created again!

    How is this possible?
