Table of Contents Missing for Certain Posts

Hi Michael,
Great plugin! The issue I am experiencing is that not all posts are showing the table of contents. All posts are set up the same, with the same number of headings, which are set to h2.
Examples:
Works: http://glutenfreeworks.com/health/2013/04/29/bitots-spots-3/
Does not work: http://glutenfreeworks.com/health/2013/04/23/iron-deficiency-anemia-3/
Please advise. Thanks!
