We want to display a certificate at the end of a survey? We do trainings in person, & want to use an online survey option to send to attendees that ends with a certificate of completion for the training that they can print or save.

If you are using the Pro version of Formidable you can use our Views system to generate an on-screen certificate after the completion of a form, and the View can display options submitted via that form: https://formidableforms.com/knowledgebase/create-a-view/

There is also a 3rd party add-on that allows you to generate PDF files from form entries: https://e2pdf.com/
