# Survey form

 * Can you create a survey form with many questions using Contact form 7? If so,
   is there a tutorial because I tried to create one and it didn’t work.

    ​ ​Yes you can definitely create a CF7 survey form with many questions.
   I don’t know of a tutorial, but if you can describe in some more detail how “
   it didn’t work” for you, I should be able to point you to some useful information.

 * Okay, so I went to create the questions, they’re embedded on my site, but when
   I go to fill them out as a test, after submit it says, “Failed to send your message.
   Please try later or contact the administrator by another method.”
