Survey fields not loaded when using Convio

    We are using Convio and the form fields are loaded in the admin survey page, but when I use the shortcode the fields are not being loaded in the front end.
    I’m using a wp 5.8.1 clean installation with default theme.
    Any idea about this issue?

    It was a JS error with captcha field rendered as undefined. We found the issue and added a field validation to avoid rendering the captcha field, but as our form has a captcha field as required we need that part of the code working.
    Can you help us with this?
