Super confused setup :/ PLEASE HELP

I developing a ratings and review site, I am trying to set up four free membership levels where each level will have access to different areas and/or options on my site. For example, levels 1, 2 & 3 (customers) will be able to write reviews while level 4 (business owners) will not but they can manage their listing.
Anyway, I “think” I have the codes correct but while people can register, they are not being registered on the level they are supposed to be on. For example, I have a drop down menu on my homepage, it says register as… and when you hover it shows you the options. I’ll use level 1 as my example if the person clicks patient or client they get this form which has the following code [s2Member-Pro-Stripe-Form register=”1″ level=“1″ ccaps=”” desc=”Patient or Client=”eatingdisorderstreatmentreviews.org” tp=”0″ tt=”D” captcha=”clean” /] once they hit submit they are able to use the site but all members are being registered at the 0 level or “subscriber”.
Can anyone help me figure out what I’m doing wrong PLEASE this has had me tied up for days and it’s REALLY getting old!
