Success message and e-mail not working

Hello, I am very new to Word Press so I may be missing something simple here.

When I first created my form, after clicking submit a success message would appear. However, this feature has all of the sudden stopped. After clicking submit it just stays on the application form page. I have tried it outside of review mode and have also tried to fix it under the processors option, but I still run into the same problem.

Also, after clicking submit the application is not being sent to my e-mail.
