SUBMIT does not do anything

    Good day,

    Before purchase I’ve been trying out the free version. I cant seem to get it to work properly and I believe my theme might be at fault. The SUBMIT button does nothing, it only reloads the page.

    There is no change in URL whatsoever. Please have a look here: https://pastebin.com/BwFT0nAn

    Any advice is appreciated.

    The shortcode I’ve configured:
