 * I was tweaking my form tonight and the submit button no longer works. Is it my
   code or the app?

    Do you is using a mailchimp plugin to this form? or is it the integration
   with Mailchimp of popup premium?
 * Well, you need enabled the name field in the plugin or if you not wish this field,
   them you must go to your mailchimp settings and put the name field as not required.
