Submit Button is not Trigerring

Hi
I am using the contact form here http://thirtyfive.io/website-onboarding-form/

But unfortunately the contact form submit button is not triggering the action..

Where it is working in the same website in this page http://thirtyfive.io/request-a-quote/

Even I tried to creating a new form that with default settings and still no luck..
