Copied “Contact Form with Captcha” to new form and changed colors only. Set email to my personal email for testing. When entering and submitting, the email comes into my inbox:
    From: WordPress
Subject: NSSDP Contact Us with Captcha

The information: email, Name, Subject, Comments are all listed in the email. I guess I was expecting it to work differently with the email actually stating the SUBJECT in the email address selected to receive submissions and the sender’s name in the FROM field.

    Dear laskowv,

Unfortunately there is not a functionality of connecting your form fields input text to form email. However, there are some fields which you can set yourself. There are “From Name” and “From Email” fields in Form Options. Please note that the subject by default is set to your form name. Please also note that if you mark the field “Send mail to submitter” From Emal field will be the email address typed by user in the form.
