 * I am considering this plugin and want to know if submissions are only send via
   email?

 * The plugin sends the submissions to email and displays in the plugin Submissions
   section. You can enable to option to save the form you want to database as well.
   Simply click on the created form, go to Form Settings and check Save Submissions
   to Database option.
