sub categories shown incorrectly

Hello
I cant seem to create sub categories correctly , when i do , the sub categories created in the second level sub category , are all placed together in on header , for example usa new york , new york city Bronx, New York (Manhattan), Queens, Kings (Brooklyn), and Richmond (Staten Island). And not just Queens (Q and A) . it gives all q and a for all sub groups together.
Also is there a short code to specify for sub group , to be placed on page
