    It’s your theme issue. Try that on twentytwelve theme.

    yes, it works correctly on TWENTYTWELVE but I can’t change theme,
    can you help me to solve it?
    Should I edit CSS?

    i m unable to use this plugin as i m newby to wordpress. please tell me how can i use the custom fields in a page after adding them. i also want to use these fields in my template file.so how can i call them in template file.

    then you add custom fields in plugin settings, you must write this shortcodes in your page content text or post content text:
    [wpuf_addpost] for add posts
    [wpuf_edit] for edit posts
    and etc. custom fields show with the main form.
