What theme error are you mentioning?
The console shows only js errors, your WP instance is not loading the site_script.js which is the main script for the store locator. To confirm it press Ctrl + U to view the source of the page, and search for site_script.js which is the required file, the console is showing no error because there was no file loaded at all.

Right. there was wp_head missed.
I added now wp_head().

Sorry, I don’t know about site_script. I guess it is plugin js.
Should you need to login in via FTP and solve the issue??
