Spinning Arrow on Contact Form 7 – ONLY on one form

    I have two contact forms on my Website (www.polymerohio.org). On the Contact Us page, the form works fine.

    On this page, however, I get a spinning arrow that never submits:

    I have read the previous solutions. Since the form works on one page, but not the other, I seriously doubt that it is a plugin conflict – having said that, I did try to deactivate the plugins to no avail. I don’t see much point in switching themes since it works on a different page.

    I previously posted a different issue about this form but since no one responded, I tried to recreate the form. This is the resulting issue now.

    Please don’t provide a solution that is too technical. My knowledge base barely includes javascript and css.
