I think that your theme may be letting the registrations through. The plugin never sees them. The captcha only shows when the plugin things a user is a spammer. If the plugin does not see the the spammer then it can’t show the captcha.
You should look at the log report and see if stop spammers is reporting anything about the registrations.

The plugin is detecting the spam, but the form sees the registration first and registers the users before the plugin has a chance to stop them. Then the plugin tries to stop them, but it is too late. They have already registered.

I don’t know what the theme is doing on the custom page. It looks like it hooks into wordpress and grabs the form so it can do the registration. If I had the theme, I could possibly find and fix the problem, but this is really the responsibility of the theme author.
Stop Spammers is behaving well and using standard wordpress events to inspect a registration assuming it is coming from standard wordpress registration forms. I have a few tweaks that let it work with gravity forms and a couple of other popular plugins.
If you can’t live without the theme, you may have to live without Stop Spammers. I looked at the WPLMS features and it seems to be a great theme with a rich feature set. It also has an active support forum. I would ask there for help with this issue. It may be an easy fix. Explain that Stop Spammers is hooking the “init” event, but the theme is getting there first before stop spammers can stop the user from registering.

thank you for your fast reply, if i get rid of this by changing the theme i will, its a great theme ofcurse but i wont but security and taking control of my site is more important, So i will clean install wp again without any plugin/themes, only your plugin.

Ok, now i have deleted everything, fresh install of wordpress and your plugin, same as with the theme, 100 block and still have users registered
Make sure that you go to the cache item under stop spammers and clear the cache. The spammers can be getting in that way.
There may be other issues.
In the log report under Stop Spammers, the registered users should appear. Check their IP addresses on the stop spammer diagnostics page to see why they were not blocked. – Copy the IP address from a user that registered on the report page and paste it into the ip address text box on the diagnostics page and press enter. This will perform all tests and show you why this IP was allowed or blocked.
