I see some markup from “All In One WP Security & Firewall” in the comment form. Maybe this is the culprit here.
Have you tried to comment yourself? Does it work?
It looks like our counter is increased and for the further actions (mark as spam, send mail, save spam reason) it is broken. So I think there is someone else fiddling with the same filters/actions. The filter pre_comment_approved
(mark as spam) and the action comment_post
(send mail and save reason) seems to be exited before our code runs and therefor our actions are not executed.
Please try to disable all other plugins (or at least “All In One WP Security & Firewall”) and try to comment. Does it work now?
Edit: You can test the spam detection if you post a BBCode link. This should mark the comment as spam. But attention: You need to un-spam it afterwards, so that the local spam db feature does not mark your comments as spam in the future.
The country blocker seems to be unrelated and if other mails are working, there is no general problem with mailing and we use the core mail function, so this shouldn’t be a general issue.
Additionally you can look at the discussion settings. Are there settings for the blocklist which could block comments?

Please report back with your findings. Maybe we can fix this together with the other plugin if there is a conflict.

Dear Torsten, thank you for the perfect analysis! I was afraid to disable the WP Security plugin completely because it tweaks so many things (like login address) and I have high number of attacks incoming (as everyone these days). However I disabled several features in the security plugin that seemed to have something to do with comments. And Voilà spam is now being stored with a reason from ASB and e-mails are coming. Now I will slowly begin to enable the fields back to isolate the exact field that was the problem. When I find it, I will post it here but it will probably take many days 🙂

Hi, I am not exactly sure, but it is probable that “Add Captcha to comments form” is to blame. (That is settings in All In One WP Security & Firewall plugin)
