Hi @kentog,
Since the wp-content/uploads/ folder is the default WP folder the images you have stored in there from your media library would be optimized when you use Bulk Smush from Smush > Dashboard > Bulk Smush panel.
If you’re looking to optimize images in a specific folder you can use Smush > Dashboard > Directory Smush option.
If this doesn’t help please provide us a bit more information on which images exactly are you looking to optimize and how the folder that stores them was crated.

It seems like all the other files in teh folder are smushed but I get this on some of the when I run this

Those /upload/2021 are the images located at WordPress > Media Library.
It seems all are the Full-Size image, for example, if it was a thumbnail the URL would look like:
https://safetyment.com/wp-content/uploads/2020/12/7-150×150.jpg
By default, Smush Free only compress the thumbnails versions and the original image ( Full size ) can be compressed only using Smush Pro.
