"Site Type" form not appearing in the dashboard

Hi. I have Membership Premium, and when I go to the main dashboard I’m not seeing the form that allows what kid of site I want. This is the form that has radio buttons for ‘standard membership,’ ‘dripped content,’ and ‘advanced’ What do I need to do to activate this?
