Simple Sign Up Sheet – Auto Post on Site?

I have created a webpage (below) for my fraternities alumni group and I need a simple plugin that will allow them to sign up for a list and then auto post it on the same page. The list would just contain name, email and phone number. Any ideas or plugins?
