Simple form?

Hi there,

I want to create a site with WordPress for competitive runners.
I want users to be able to fill out a simple form with only a few fields like name, lap time, date and race track. The data from each field then gets added to a page or post below any other runners’ lap times and so on until I would then end up with a list of all the entries.

Can anyone suggest a plugin that would help me achieve this?
