Sidebar optin form not appearing

    Just downloaded this plugin for wordpress.org page http://www.reallifenaturalwife.com

    I successfully got the scrollbox optin form to appear and have already gotten some subscribers within first few minutes.

    I am not able to get the sidebar optin form to appear. I went through the steps just the same as I did with the scrollbox optin but it does not appear on my sidebar.

    I went to appearance-widgets and added it to the sidebar. Still no luck with it appearing.

    Please help me make the email optin form appear on my sidebar.
