Show Form when Contact is clicked

A.] How can I show a “contact seller” button below each listing and when a user clicks it, a form pops up to get the details of the buyer?

OR

B.] When a buyer initiates a Chat with a seller, is it possible to show a form to get the contact details of the buyer (probably in the chatbox) before the Chat then proceeds?
