Show form fields based on date

I have a form where a serires of fields need to show based on what month of the year it is. I have PHP if/then code working just fine for a ‘normal’ form, but it doesn’t work when I put that code into a ‘Contact Form 7’ form. Can anyone show me how I can make this work?
